About Us

The adult services team deliver all aspects of patient care for adults in a community setting. This can be in the home, a care home or health centre.

Our teams are made up of highly skilled nurses, therapists and practitioners who deliver district nursing services, urgent care services within the community and intermediate care teams that provide a short term rehabilitation service. Our practitioner led services reflect the diversity of our populations health needs with treatment and care for anti-coagulation, DVT, chronic pain management and TB services being delivered in the community setting.

Our award winning end of life health and social care team work alongside our other services to ensure patients in an end of life situation receive the highest quality care and support at such a challenging time.

Innovation is at the forefront of our adult services and whilst we continue to deliver our core services we continue to expand our portfolio by rolling out Tele-health and exploring new business areas such as a carers’ support service for Hull.
