Cardiac Rehabilitation - Hull & East Riding
What is the Service?

The Cardiac Rehabilitation Service consists of nurses and a physiotherapy team with specialist knowledge of Coronary Heart Disease and Cardiac Rehabilitation. They team work across Hull in conjunction with GP’s, practice nurses and other health professionals in the development and provision of cardiac rehabilitation and coronary heart disease management.

In practical terms, cardiac rehabilitation is a professionally supervised programme of:

- An initial patient assessment to determine risk factors, patient’s needs and limitations.

- A comprehensive cardiac rehabilitation programme consisting of:

  • Education and health behaviour change.
  • Lifestyle risk factor management.
  • Physical activity and exercise.
  • Diet.
  • Smoking Cessation.
  • Psycho-social health.
  • Medical risk factor management.
  • Cardio-protective therapies.
  • Long-term management.

Aim of Service

The service aims to:

  • Provide a high quality service that offers patient centred and clinically effective cardiac rehabilitation services to patients following the diagnosis and treatment of a cardiac condition such as a heart attack, angioplasty, coronary artery bypass or valve replacement surgery, defibrillator implant or following the diagnosis of angina or heart failure.
  • The service support the medical management of this client group and ensures a seamless service between the acute and primary care setting, along with the provision of education and support to colleagues within CHCP.

Service Objectives

  • To provide a high quality, efficient evidence based cardiac rehabilitation programmes for those people who have experienced a cardiac event.
  • To provide a patient centred cardiac rehabilitation service which will meet the personal needs of each patient.
  • To ensure that patients and carers have appropriate information allowing them to manage their own care more effectively.
  • To provide seamless, co-ordinated care through close collaboration with partner agencies.
  • To ensure the service is readily accessible among diverse patient groups.

Service Referral - How to Refer and Criteria

Patients will be referred to the service by the hospital or consultant cardiologist, please call 01482 335466 for further referral information.

Referral Criteria - Hull

  • Patients ages 18 years or older registered with a Hull GP
  • Recent cardiac event such as coronary bypass surgery, valve replacement, valve repair, heart attack, Takotsubo cardiomyopathy, exertional angina, heart transplant, mild heart failure, defibrillator devices and/or cardiac resynchronisation therapy devices

Referral Criteria - East Riding

  • Patients aged 18 years or older registered with an East Riding GP
  • Recent cardiac event such as coronary byass surgery, aortic or mitral valve replacement(s), aortic or mitral valve repair, a heart attack and/or heart failure.

Exclusion Criteria

  • Unstable angina.
  • Unstable arrhythmias.
  • Other complex medical needs e.g. life threatening co-morbidities.
  • Patients under the age of 16.

Expected Outcomes for the Service

  • All patients who have experienced a heart attack, angioplasty, coronary artery bypass or valve replacement surgery, defibrillator implant or following the diagnosis of angina or heart failure will offered a cardiac rehabilitation programme.
  • Reduction in inappropriate admissions with chest pain.
  • Reduction in Cardio Vascular Disease mortality rates.
