What is the Service?

The Continuing Care Team consists of Registered Nurses and Administration support. The team consider applications following a multi-agency assessment of an individual’s needs. They work within a national and legal framework when determining eligibility for fully funded NHS care, the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, June 2007, revised October 2009.

Fully funded NHS care describes a package of care arranged and funded solely by the NHS and the services are free to the individual. This care can be provided in any setting including in the individuals own home.

Registered Nursing Care Contribution (RNCC) or NHS funded nursing care as it is sometimes referred to, is a health contribution based on an individual’s nursing care needs, and for those individuals whose needs have been assessed as and require a Care Home registered for Nursing and do not meet the eligibility criteria for fully funded NHS care. This is reviewed on a regular basis and at each review; the individual is screened against the eligibility criteria for fully funded continuing care.

Aim of Service

The service aims to:

  • Support and work in partnership with the wider health, social, voluntary and independent teams to ensure that the individuals needs are met locally and are cost effective.
  • Manage patients who have long term complex health and social care needs and who meet the eligibility criteria for fully funded NHS care.
  • Provide education and support for Health Care Professionals, patients and carers, local authority, private and voluntary agencies.
  • Provide input into the strategic and national framework for fully funded NHS care and NHS funded nursing care.
  • Facilitate patient choice wherever possible.
  • Review individual’s eligibility for fully funded NHS care, monitor care packages and placements ensuring that their needs are appropriately met and cost effective.

Service Referral - How to Refer and Criteria

Referral Criteria:

  • Eligibility for fully funded NHS care is based on care needs not diagnosis.
  • Individual registered with a Hull GP for at least six months.
  • Identified as having complex health and social care needs.
  • Adults aged 18 and above, although young adults coming through transition from
  • Children’s services can be referred.
  • Completion of a continuing care checklist that triggers for further consideration.

Criteria for fast track referral:


  • Need to be assessed as requiring care within a nursing home setting.
  • Nursing home needs to be within the NHS Hull area for those who require permanent care.
  • For respite, need to be registered with a Hull GP.
  • Patients who have a rapidly deteriorating condition and may be entering a terminal phase of illness.

Expected Outcomes for the Service

To ensure that all individuals who are eligible for Fully Funded NHS and Registered Nursing Care and who are the responsibility of CHCP, are assessed equitably against the relevant criteria and that the funding identified is appropriate to meet their needs and is cost effective.

The introduction of personal health budgets for patients eligible to continuing health care funding.
