Intermediate Care Services

What do we do?

Intermediate Care is a short-term rehabilitation service provided either in a 24-hour care environment or in your own home. It's free at the point of delivery.

Health and social care professional work together to support you in situations where you are suddenly unable to safely manage everyday activities due to illness or injury.

Who can use the service?

The service is available to people aged 18 and above who have a condition that is stable and can be managed safely in the community.

What do we provide?

A wide range of services such as physiotherapy, occupational therapy, district nursing, community nursing and assistance with personal care as needed. All professionals involved in your care attend weekly team meetings to keep up-to-date on your progress. Care co-ordination will oversee your care.
