What is Telehealth?

The Telehealth service is offered to support individuals living with their long term condition from within their own homes allowing them to live more independently in their own homes for longer. These conditions can range from Chronic Obstructive Pulmonary Disease and Heart Failure. This service can be provided in conjunction with Hull Churches Home From Hospital Service who can offer patients with additional support in using their telehealth equipment.

Telehealth is a simple user friendly device designed to monitor your health on a daily basis and identify any deterioration before it becomes critical, reducing your GP visits and the need for unnecessary hospital admissions. The device is programmed to take your readings such as oxygen levels, weight, pulse, temperature and blood pressure depending on your condition. The device will also ask you some questions about your general health and well-being. Patient’s readings are then transferred automatically from the device using a telephone or internet connection to specially trained Assistant Practitioners in our Telehealth Hub. If any readings are outside of the patient parameters, your Assistant Practitioner will contact your Community Nurse.

Service referral

To use Telehealth, you should have a primary diagnosis of Chronic Obstructive Pulmonary Disease (COPD) or Heart Failure.

  • Be over 18.
  • Be registered with a Hull based GP.
  • Patients with a chronic disease/long term condition who have complex needs which can be health, social or both.
  • Frequent requests for GP attendance due to chronic disease/long term condition.
  • Poorly managed or unstable chronic disease/long term condition.
  • Acute exacerbation of chronic disease/long term condition.
  • Frequent user of emergency and/or Out of Hours services due to chronic disease/long term condition.

Exclusion criteria

  • Patients under 18.
  • Patients not registered with a Hull GP.
  • Have another chronic condition that dominates their care.
  • Have received a transplant of a heart, or other significant organ.
  • Be in the terminal stages of illness.
